Getting Started

Applying for placement:

The first step is to complete and submit the Long-Term Care Application and Financial Worksheet along with all other requested documents. You may do so by going to:

Bed Availability:

We recommend the application materials are submitted even if you’re not ready for admission. As beds become available, we contact applicants to make bed offers and having the application in hand will expedite the process. If a bed is offered and you’re not ready, we put the application back on file for future calls.

Tours:

Tours are scheduled by appointment only and are generally arranged to occur mid-week. To schedule a tour, please email LTCApplications@BroadReachHealth.org.

Departments & Services

Residents at Liberty Commons receive around-the-clock care and support from a dedicated team of professionals. The team includes multiple members across varying departments, those of which all have the shared goal of providing the highest quality, individualized care.

Clinical:

  • Attending Physician – All residents are assigned an attending physician.
  • Nurse Practitioner – The nurse practitioners (NP) will continue to provide more of the day-to-day oversight of medical care and the physicians will round regularly to supplement the NP visits.
  • Nursing – A team of nurses and aids provide 24/7 care to the residents in the building.
  • Optional Medical Services – Podiatry, Dentistry, Optometry, and Behavioral Health Care are available (services will be billed to resident’s health insurance according to provider/plan guidelines).


*Durable Medical Equipment - Provided by resident/family (walker, wheelchair, etc.).

*Prescription Medications - Medications are provided through our contracted pharmacy and billed to the resident’s prescription coverage provider per plan guidelines (co-pays, etc.).

Rehab:

  • Physical, Occupational, Speech, and Language Services – May be provided depending on medical necessity.


Social Services:

  • Residents and their families are assigned a licensed social worker who will be available as a primary point person throughout the duration of the stay. 


Recreation/Activities:

  • A variety of specialized activities and recreation programs are provided for all residents and include special events, seasonal celebrations, summer cookouts, live music, and more.

Dietary Services:

  • Daily meals – Residents are provided 3 main meals, as well as various food/beverage options throughout each day.
  • Therapeutic diets - Available along with adaptive utensils/dinnerware.
  • Dietician – A dietician is on staff to ensure residents are provided with healthy, balanced meals

Administrative:

  • The office staff at Liberty Commons is available to assist residents and families with various inquiries ranging from phone extensions to billing.


Environmental & Housekeeping:

  • Maintenance and housekeeping staff play an integral role at Liberty Commons in ensuring our facilities and grounds remain well-maintained and clean.

Additional Services and Amenities

  • Television – TV provided by resident/family, no cost for cable TV.
  • Wi-Fi – Residents and guests enjoy complimentary WiFi throughout the building.
  • Telephone – Phone lines are available for a monthly fee. Residents may also use their own mobile phone.
  • Laundry Services – Provided at no cost - residents’ clothing items are labeled by our laundry service
  • Toiletries and Incontinence Products – Basic toiletries are provided at no additional cost.
  • Salon – Costs are dependent upon the desired services.
