How To Find Us

To Liberty Commons, Broad Reach Outpatient Rehab Clinic and The Victorian (389-390 Orleans Road, North Chatham):

  • Route 6 (Mid-Cape Highway) East or West to Exit 85 (Formerly 11)
  • Left off the exit ramp onto Route 137 South
  • Immediate left onto Pleasant Bay Road
  • Follow Pleasant Bay Road through the stop sign to its end at Route 28 (by the Wequassett Inn)
  • Right onto Route 28
  • Follow Route 28 into Chatham. Our properties are located shortly after Ryder’s Cove on both sides of the street.
  • Liberty Commons & Broad Reach Rehab are on the left.
  • Broad Reach Assisted Living at The Victorian is on the right.

To Broad Reach Hospice (1455 Main Street, Chatham):

  • Route 6 (Mid-Cape Highway) East or West to Exit 85 (Formerly 11)
  •  Left off the exit ramp onto Route 137 South to its intersection with Route 28
  • Turn left onto Route 28
  • Arrive at 1455 Main Street, just past Ocean State Job Lot – Broad Reach Hospice building is on the right.
